Public health touches all parts of daily life, and your responses to a new survey, the State Health Assessment, will help shape our state’s priorities moving forward.
Tell policymakers any issue you think is worth addressing on a statewide level. Topics like tobacco use, health care access, environmental issues, opioid misuse and overdose, homelessness, disability, car and traffic safety, and anything else are welcome.
The survey is completely voluntary and no identifiable information will be collected. Do not put your name, address or any other information that could identify you in your answers. This survey is not intended for people under the age of 18. If you are under the age of 18, please ask permission from a parent or guardian before filling out the survey, or have them complete it on your behalf.
While this survey is anonymous, information collected via this survey may be subject to release in accordance with RCW 42.56 (Public Records Act).
Background:
The purpose of a State Health Improvement Plan (SHIP) is to decide on a set of statewide health priorities and develop plans for improving each priority.
Priorities are determined by a team of partners from across the state.
While data from the State Health Assessment is included in the decision-making process, the SHIP team is also interested in hearing feedback from all Washingtonians on what priorities should be included in the SHIP. (Taya Briley)
