Hospitals now have until Oct. 1 to complete the Washington State Department of Health’s Acute Care Bed, Operating Room Utilization and Home Health Utilization surveys. These surveys play a vital role in evaluating current service use, forecasting future needs and informing statewide health planning. Your input helps ensure that health care resources are aligned with community needs across the state.
Acute Care Bed Survey:
- Complete the online survey (one per agency), or
- Submit the Acute Care Bed Response Workbook with data for all agencies to CN@doh.wa.gov.
OR Utilization Survey:
- Complete the online survey (one per agency), or
- Submit the Operating Room Response Workbook with data for all agencies to CN@doh.wa.gov.
In addition to the three surveys with deadline extensions, the 2025 Home Health Utilization Survey is now live. This survey also has a deadline of Oct. 1. If your organization provides home health services, please submit prior to the deadline. The home health utilization survey has two options for data submission:
- Complete the online survey (one per agency), or
- Submit the Home Health Provider Response Workbook with data for all agencies to CN@doh.wa.gov.
For all surveys, DOH recommends having the following information available:
- Your Department of Health issued license number.
- Your facility’s contact information.
- 2024 change of ownership info (if applicable).
- 2024 data on procedures and OR use.
