WSHA invites hospitals to participate in a new DELTA Collaborative focused on reducing hospital readmissions among adult Medicaid Managed Care Organization patients. This is a key quality metric in Washington’s State Directed Payment Program.

The Plan All-Cause Readmission rate is one of four measures the Centers for Medicare & Medicaid Services expects states to improve. With future directed payments tied to measurable progress, the work is more important than ever.

The DELTA Collaborative is WSHA’s most intensive training approach, designed to help hospitals implement real-time safety and quality improvements. Over 18 months, participants will engage in shared learning, front-line collaboration and rapid-cycle change. The program begins with a virtual pre-kickoff meeting on Aug. 19 at 10 a.m., followed by an in-person kickoff on Sept. 19 from 9 a.m.-to 3 p.m. at WSHA’s Seattle office.

Following the kickoff, participants will join ACCELERATE webinars: structured workgroups providing support and on-going collaboration.
